Feedback Form
To all members,
WMHA is now offering a feedback form for this past season. Please take some time and fill out this form before July 25 2015. The purpose of this Parent Feedback is to gather feedback which will help the Walkerton Minor Hockey implement and continuously improve programs which encourage and support long term player development. The feedback will also assist in identifying the strengths and areas of opportunity related to the delivery of WMH programs. The Feedback will be reviewed by the WMH Board. All feedback forms will remain confidential. This will allow you to be direct and honest with your feedback. If you wish to provide feedback for multiple players/ teams, please complete individual forms.
