Mandatory Police Checks

Hello Everyone,

This year Walkerton Minor Hockey requires ALL coaching staff and bench staff to perform a mandatory police check. Walkerton Minor Hockey will be doing this one night only. The night will be October 7th at the arena from 6pm to 9pm.




Each coach or bench staff needs to show two pieces of approved and valid pieces of identification. We will be making copies of this identification. At least ONE piece of identification must include a photograph of the volunteer, taken within the last 5 years.

The photo identification shall contain, at a minimum, the Subjects full given name, surname, date of birth and gender.

Acceptable Photo Identification:

Driver's licence
BYID Card
Canadian or Foreign Passport
Canadian Citizenship Card
Possession and Acquisition Licence (or PAL card) Permanent Residence Card Certification of Indian Status Canadian National Institute of the Blind (CNIB) Indentification Card Canadian Military Employment Card or Canadian Military Family Identification Card Ontario Photo ID Card

Acceptable Forms of Non-Photo Identification:

Birth Certificate
Baptismal Certificate
Hunting Licence
Outdoors Card
Canadian Blood Donor Card
Immigration Papers
